New Library Cards
In order to receive a library card, a new patron must
1. Fill out the application form
The application form is a page long, and asks for basic information such as name, phone number, and mailing address. We also require that all patrons have a valid driver's license number or state-issued ID for the form as well, when applicable.
2. Bring in proof of address
Proof of address is basically any object that proves the address on the application is where the person lives. This can be a piece of mail, a bill, or anything else with the patrons name and home address.
We do issue temporary cards for out-of-town family members staying long-term in Beloit or the surrounding area.
We issue cards to people living in area towns and communities, or those regularly traveling to Beloit. (Cawker City, Jewell, Glasco, Simpson, Jamestown, Concordia, Downs, Tipton, Minneapolis, Osborne, Superior, Salina, etc.)
The first time a patron checks out, they are limited to two items per registration. After successfully returning those items, patrons are free to check out as many books as they want. There are other limits for audio and visual materials. See this page for details.
When a child is getting a library card, at least one parent/guardian will need to sign the child's registration and register for their own card as well.
Registration forms for children or adults can be downloaded via the link below (Microsoft WORD format.)
